Coaches partner with patients to support behavior change, self-management, and overall well-being.

we help patients bridge the gap between medical advice and daily life.

Refer a Patient for Coaching:​

Health and wellness coaching benefits patients who are:

  • Medically stable but need support implementing lifestyle or behavior changes

  • Express readiness or interest in improving areas like: Nutrition, Physical activity, Sleep hygiene, Stress reduction, Time management / work-life balance

  • Struggle with motivation, follow-through, or accountability

  • Are living with chronic conditions (e.g., diabetes, hypertension, autoimmune disease) and need help with lifestyle management

  • Are working toward mental health maintenance and could benefit from structure and support between therapy sessions.​

Patients are not ready for health and wellness coaching if they:

  • Are in acute medical or psychiatric crisis (e.g., suicidal ideation, severe depression, unmanaged psychosis, unstable medical condition)

  • Require interventions outside the coaching scope such as clinical diagnosis, treatment planning, or psychotherapy

  • Are not interested or ready for behavior change

Health & wellness coaches do not diagnose, treat, or replace medical or mental health care. Coaching complements care by supporting behavior and lifestyle changes aligned with your treatment goals.
